To request a name change, please complete the Name Change Only application through the Louisiana Educator Portal. Include the required supporting documents requested on the application. Once all forms in the application are complete, select Check Out from the left navigation menu. A $25 processing fee is required when requesting a name change only. Pay the processing and Submit the application.
A name change can also be requested on any application for certification or change to a certificate that requires a processing fee. If requested on the Personal Information form in other applications requiring a fee, the name will be changed when the other certification request is processed. No additional fee is required to request a name change with other paid requests.
Name changes cannot be requested on any application that does not otherwise require a fee (World Languages Certificate, Early Childhood Ancillary Certificate, Resident Teacher). Educators holding these certificates and wishing to change the name should use the Name Change Only application.
Our office can only change a name to the legal name reflected on the social security card provided. An updated social security card reflecting the new name is required with any request to change a name.
Please note, a certificate must be valid to make any changes, including a name change.
